1. Acceptance of Terms
These Terms of Service ("Terms") govern your use of the website located at [www.12housewellness.com] (the "Site"), owned and operated by Twelfth House Wellness, LLC ("Twelfth House Wellness," "the Practice," "we," "us," or "our"). By accessing or using the Site, you agree to be bound by these Terms. If you do not agree, please do not use the Site.
2. Purpose of the Site; No Therapist-Client Relationship
The Site is provided for general informational purposes about the Practice, its clinician(s), and the services offered. Content on the Site — including blog posts, articles, and descriptions of therapeutic approaches such as Acceptance and Commitment Therapy (ACT), somatic work, or transpersonal frameworks — is educational in nature and is not therapy, clinical advice, diagnosis, or treatment.
Browsing the Site, submitting a contact form, or otherwise communicating with us through the Site does not, by itself, create a therapist-client or counselor-client relationship. That relationship begins only after mutual agreement to engage in services, completion of intake paperwork, and execution of a separate Informed Consent for Treatment.
3. Not for Emergencies
The Site and any contact form, email, or voicemail associated with the Practice are not monitored continuously and must never be used for emergencies. If you are experiencing a mental health crisis or medical emergency:

● Call 911 or go to your nearest emergency room
● Contact a local crisis line or mobile crisis team in your area
Twelfth House Wellness does not provide emergency or crisis intervention services through this Site.
4. Eligibility and Location of Services
Twelfth House Wellness provides clinical counseling services to residents of, and to clients physically located in, the states where the clinician is licensed and legally authorized to practice: currently Oklahoma (LPC# 7013) and Oregon (LPC# C11541). Services are delivered via telehealth and, where clinically appropriate, in-person within Oklahoma primarily.
It is the client's responsibility to inform the clinician of their physical location at the time of each session, as this determines which jurisdiction's laws and licensing rules apply. Services are not available to residents of, or persons physically located in, any state where the clinician is not licensed or otherwise legally authorized to practice, without prior written confirmation from the Practice.
5. Scheduling, Communication, and Client PortaL
Appointment requests, intake forms, and secure messaging are handled through our client portal and scheduling tool [Simple Practice & Quenza LLC]. Use of these third-party platforms is subject to their own terms and privacy practices, in addition to this Policy and our Notice of Privacy Practices.
Standard email and website contact forms are not appropriate for sharing protected health information (PHI) and should be limited to general, non-clinical inquiries (e.g., asking about availability or fees).
6. Non-Clinical Offerings
From time to time, the Practice or its owner may reference non-clinical offerings (for example, community based groups, experiential groups, support groups, or any other astrology or tarot/oracle readings offered under a separate brand identity). Any such non-clinical offerings are not psychotherapy, are not provided within a clinical or counselor-client relationship, are not billable to insurance, and are not subject to the clinical confidentiality protections described in our Notice of Privacy Practices. Where such offerings are described on this Site, they will be clearly labeled as separate and non-clinical.
7. Fees, Cancellations, and Payment
Fee schedules, cancellation policies, and payment terms for clinical services are set out in your individual Informed Consent and/or client agreement, not in these Terms. General fee information published on the Site is for informational purposes and is subject to change without notice.
8. Intellectual Property
All content on the Site — including text, graphics, logos, the "Twelfth House Wellness" name and mark, and any original frameworks, worksheets, or materials — is owned by Twelfth House Wellness, LLC or its licensors and is protected by copyright and trademark law. You may view and share Site content for personal, non-commercial purposes, but may not reproduce, distribute, or create derivative works from it without prior written permission.
9. Third-Party Links
The Site may link to third-party websites (for example, professional directories, licensing boards, or referral resources) for convenience. We do not control and are not responsible for the content, accuracy, or privacy practices of third-party sites.
10. Disclaimers
THE SITE AND ITS CONTENT ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE.
11. Limitation of Liability
To the fullest extent permitted by law, Twelfth House Wellness, LLC and its owner, employees, and contractors shall not be liable for any indirect, incidental, consequential, or special damages arising out of or relating to your use of the Site. Nothing in this section limits liability for clinical services, which is governed separately by your Informed Consent and applicable state law.
12. Indemnification
You agree to indemnify and hold harmless Twelfth House Wellness, LLC from any claims, damages, or expenses (including reasonable attorneys' fees) arising from your misuse of the Site or violation of these Terms.
13. Governing Law
These Terms are governed by the laws of the State of Oklahoma, without regard to conflict-of-law principles, except where superseded by mandatory Oregon law applicable to clients located in Oregon.
14. Changes to These Terms
We may update these Terms from time to time. The "Last Updated" date at the top of this page reflects the most recent revision. Continued use of the Site after changes are posted constitutes acceptance of the revised Terms.
